Near-miss reporting system

United States firefighters use an informal reporting system for accidents that didn’t quite happen.  It is an interesting approach to learning from what they call “unintential unsafe occurances”.  It is a way to learn from real life events that could have been tragic.

“The National Fire Fighter Near-Miss Reporting System is a voluntary, confidential, non-punitive and secure reporting system with the goal of improving fire fighter safety.”
